Northside Gastroenterology is committed to protecting the privacy of our patients and the confidentiality of their personal and health information. We manage personal information in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs) and applicable Australian Capital Territory health records legislation.

What information we collect

The information we collect and hold depends on your relationship with the practice, and may include:

  • your name, date of birth, address and contact details;
  • Medicare, health fund and other billing details;
  • health information contained in referrals from your medical or nurse practitioner;
  • health information you provide to us, or that is generated in the course of your care, including consultation notes, procedure reports and test results;
  • the name and contact details of your referring and nominated healthcare practitioners.

How we collect information

We collect information directly from you wherever practicable (in person, by telephone or through paperwork you complete) and from referrals and correspondence sent to us by your treating practitioners, including via Healthlink secure messaging.

Why we collect information

We collect, hold and use personal and health information to:

  • provide gastroenterology and hepatology consultation and endoscopy services;
  • make and manage appointments and procedure bookings;
  • communicate with your referring practitioner and other practitioners involved in your care;
  • manage billing, including Medicare and health fund claims;
  • meet our legal and professional obligations.

Disclosure of information

We may disclose relevant personal and health information to other healthcare providers involved in your care, for example your referring practitioner and the hospitals and endoscopy facilities where your procedures are performed, and where disclosure is required or authorised by law. We do not sell personal information, and we do not disclose it for marketing purposes.

Storage and security

We take reasonable steps to protect the personal information we hold from misuse, interference, loss, and unauthorised access, modification or disclosure. Access to patient records is restricted to practice staff who require it for the purposes described above.

This website

This website does not ask you to submit personal or health information, and does not use forms to collect it. Our hosting provider may keep standard, anonymised server logs (such as pages visited and browser type) for security and maintenance. The map on our Contact & Location page is provided by Google and is subject to Google’s own privacy policy.

Access and correction

You may request access to, or correction of, the personal information we hold about you. Please contact the practice on (02) 6251 0255 and we will respond to your request in accordance with the Privacy Act.
